Damage caused by water is the most common type leading to repairs of the basement or foundation of the house. There are different ways for water to get into the basement and it leads to damage. There are two common methods for foundation repair, namely, Piering and Slabjacking. In case of Piering, an underground support is provided and the concrete is lifted for fixing the structure. It is also known as anchoring and this is the method that is usually used in cases where the soil has high humidity or the concentration of water is high. The anchor can protect the foundation from water seeping in.

Slabjacking

The area underneath the basement is filled with Grout mixture in this method and this helps the concrete to regain its previous position.

Today, there are advanced foundational repairs with the use of steel rods and concrete. The use of shims and steel rods with concrete poured in can do most of the repairs, but they are more expensive. However, it is quite effective and beneficial. Additionally, many tools such as French drains and craw space vent fans are also installed in order to maintain the dryness of the basements.

    I have heard that there is a real problem with settling due to the clay soil in my area and that this can be avoided by keeping the foundation moist. And how often should you water in the winter when the Bermuda grass goes completely dormant from Nov 15 to March 15. I have a poured pier foundation and a very thick slab but I have heard there are a lot of problems with people’s foundations here and I want to do what is prophylactically necessary in order to insure that I do not have any problems.       It is important to water; otherwise your foundation can crack and you will have to deal with foundation problems and repairs. Watering the foundation every other day in the summer when the temperature gets over 90 degrees should be sufficient. I recommend using soaker hoses placed 2-3 feet away from the foundation. We have Bermuda grass also and never water it in the winter down here in Texas. All grass is suppose to go dormant in the winter; it is normal and will not cause any harm to the grass. In North Texas start watering the grass in mid-February; it’s good to start getting it moistened in advance like that. If you have water restrictions like us you may not be able to water it as often. However, even with water restrictions, in some areas one can water as often as they like with a hand held hose. I recommend contacting your City Hall or water department to see what water restrictions you have and what is allowed. If you don’t and you get fined the fines are very steep.

    My mom has some broken blood vessels on her nose, its from when she was in labor with me (my bad!) anyways, she’s pretty self conscious about them.       Knowing her skin type helps.

      I would suggest changing her skin care regimen to an Avon Anew product. There are several to chose from depending on skin type, oily, dry, age repair and hydrating/contouring. With that she should pick up the cleanser, day moisturizer with spf and night cream, eye cream. I would also recommend that she use the Anew Clinical Dermabrasion System or Advanced Retexturizing Peel and Anew Luminosity Brightening Serum in conjunction with the cleanser and moisturizers. My mother in-law had a permanent indentation and discoloration due to broken vessels from wearing glasses and over a years time, you can’t even tell.

      When it comes to a concealer and foundation duo that requires max coverage, I really like Sheer Cover mineral foundation. You can order it online and as a gift, they sometimes have a mini set for your purse. You can also customize your order. But I think they have one of the best ever concealers. Sheer Cover also has nice eye shadow pallets and a very nice all over face pallet perfect for her purse.

      I also love BareMinerals foundation. They have a separate concealer, but I think their foundation works perfectly as one. Bobbi Browns also carries a foundation that provides nice coverage.

      every state handles taxes differently.

      In general you should initially contact the assessor for your city/ county to figure out what the appeal process is.

      In most communities, the taxable value is directly (via a formula) related to the True Cash / Market Value of a property. Market value = what a willing buyer will pay to a willing seller in an arm’s length transaction.

      Bring sales prices of comparable properties to justify your position. Depending upon how much tax we are talking about, you may want to pay for a professional appraisal to make your case.

      Whether you go before a Board of Appeal, or just the local assessor, don’t let them buffalo you into accepting last year’s value or some value that has no relation to TODAY’s value. Your foreclosure buying price is going to be very close to market value.

    Water leaks out of foundation crack every time I do large loads of laundry. Is the problem external?
    The crack was sealed last November and has opened up again. Before it was sealed the water would start to leak out right after the pump would turn on to pump up any water in the sup pump pit. The crack in question is a few feet away from the sup pump. Thanks in advance.

      You have a broken sewer main . it needs to be uncovered and repaired . I would highly doubt your house will slide off of it’s foundation , but it sure sounds exciting !

    how to repair cracks on cement driveway?

    • ANSWER:
      You can buy at home depot or lowes some concrete gap filler that is grey and expandable with weather conditions. However, the best way is to Mud Jack the concrete to take care of the underlying problem which is most likely a drainage and settling issue rather than roots.

      Because of the evidence of cracking, just to be prudent, look around the foundation of your home and make sure that there is at least a 1% slope going away from your property around the perimeter so that you don’t get drainage and then foundation issues. Do this and repair any problem, especially if the cement is near your house or foundation since you don’t want water going under concrete and causing foundation issues.

    I have a 90+ year old house with crumbly, limestone foundation. The unfinished basement only leaks in extreme weather (once/year, just enough to stain the floor, but not puddle). I’d like to reclaim some square footage by turning the space into an office/living area. What moisture factors should I be aware of? How can I minimize the threat of water damage? Would finishing the attic be a better idea?

    • ANSWER:
      An important factor here is your outside drainage. Another no so important is basement ceiling height (your home’s age usually indicates a lower ceiling).

      Hopefully your drainage is such that it can be easily cleaned or updated. If part of the basement is above ground, that is you live on a hill, then you are home free. That is the best way to fix your moisture problem and because this would be a necessary fix to your home anyway, this could be considered a non cost to your project.

      Ceiling height is important for comfort, air quality and resale value. If your ceiling is lower than eight feet, consider “digging down”. Look at costs and feasibility. This would also be a repair needed regardless of the finishing.

      My preference would be to finish the basement and would do so unless either of these improvements show to be cost prohibitive.

      A number of engineering companies routinely repair sinkholes. Techniques vary from simple injection of grout into the hole to more advanced systems of engineered reinforced plugs, pins, and porous concrete. In general, if a repair has been certified by a licensed engineer, and completed to the satisfaction of the homeowner’s insurance company, it is probably safe. However, as you are dealing with natural systems, there can be no guarantees that a repaired sinkhole will not cause future problems.

    I have a large crack in the concrete foundation wall of my one storey house. How can I repair it meaningfully?
    I want to avoid making it look like a sleazy coverup! It’s visible from the ground about three feet up to the wood siding. Thanks in advance!

    • ANSWER:
      I guess I don’t seee where you said “block wall” Other than that if it is concrete there are a couple of fixes that can be used. They are called out below as quik crete, foam and or caulk.
      I am more inclined to tell you to use an epoxy material to “glue” the crack back together. The crack should be cleaned to remove dirt, dust and insects. Then after it dries put duct tape over both side of the crack to form a dam. Leave access holes about 1′ apart.
      Go to Home depot and buy Set 22 epoxy cartridge and gun and shoot the epoxy into the crack. Patch each hole with a wood plug as yyou work up the crack.
      After 24 hours you can pull the tape and apply a slurry of cement creme over the exposed epoxy to get it to blend.
      NOTE instead of Duct tape to seal the crack a superficial patch about 1/2 inch deep on each side of the crack will work better.
      If the crack is not still moving you can fill the crack with mortar or concrete depending on the width and depth. Mortar will fill a small crack and concrete mix for a wide crack . Quikcrete does make patching mortars that should match good. Same method of damn and fill only one side placing material full depth.

    I’ve had borderline high blood pressure for about a year and have made significant lifestyle changes that are bringing it down. So I’m just wondering — Are there any specific foods that will change/repair/revitalize my arteries? Is it possible to regain elasticity? I’m in my 50’s, and high blood pressure and heart disease are in my family history. Please provide studies or resources as I am also researching this for a paper.

    • ANSWER:
      You absolutely CAN halt the progression of coronary artery disease (CAD). Most of the time, it is also reversible with diet and stress management. By diet, I mean a very low fat (less than 10% of calories from fat), plant based diet free of added fats/oils/animal products/refined foods/simple sugars/processed foods/white flour/white rice. Essentially the diet is very low fat, vegan (or includes only fat free dairy items) and Is also low-glycemic.

      Dean Ornish MD is an authority in preventative cardiology and has proven that even the worst cases of coronary artery disease can be reversed or at least halted. You can find Dean Ornish online at pmri.org and he has published many books and professional articles. His groundbreaking work on reversing heart disease with diet was published in the medical journal called “The Lancet”. Ornish has also written the chapter on preventative cardiology in Harrison’s (which is the bible on which all modern medicine is based-and both his works are VERY prestigous accomplishments). Ornish has also written several books but the ones you will probably find most useful are “The Spectrum” and / or “Eat More, Weigh Less”. He is founder of he non-profit Preventative Medicine Research Institute in Sausalito CA and their website is already referenced above. Patients who are assigned to Dr Ornish had such advanced heart disease that surgery wasn’t an option. they were essentially “hopeless” and “beyond help” – expected to die of heart disease- which makes Ornish’s accomplishments all the more important. Besides, even when drugs and surgery are treatment options, most doctors never tell their patients the reality that current drugs and surgeries, including cholesterol lowering meds/stints/CABGs do NOT extend the lifespan of patients. Futhermore, they only treat the symptoms of heart disease instead of its causes. Current treatments, aside from lifestyle and dietary changes, also have lots of negative side effects and risks.

      Aside from Ornish, there is Caldwell Esselstyn MD from the Cleveland Clinic foundation who recommends a diet slightly more restrictive than Ornish (totally vegan whereas Ornish allows for fat free dairy). Esselstyn has also has lots of success at reversing CAD and enhancing the lives of patients. Look him up in Google by name and find him at Clevelandclinicfoundation.org.

      Aside the two above, John McDougall MD also has great resources and info about how to reverse CAD. He advises a similar diet plan as the prior two.

      There’s no escaping the very low fat, plant based diet. It is the only way to restore elasticity. It isn’t restrictive, it is wonderful and very easy, but people make a huge deal out of giving up a few “foods” that nobody should be eating on a reular basis anyway – if at all. And anympotential sacrifice in terms of foods is over-compensated by surviving a progressive and eventually terminal disease. The diet also has many other benefits, including cancer prevention and the lengthening of “telomeres” which are a biological measure of human lifespan (increasing telomeres=extending lifespan).       This was probably pretty much obvious. The problem is that people just don’t want to believe it is so simple. And many just want a magic pill but it doesn’t exist so just choose to eat right and you benefit with a healthy heart and enormously increased quality of life.

    I am looking at purchasing a home and we just had a home inspection done yesterday. The house has some settlement issues and needs some work.       you failed to mention what area you are in and whether you are on a slab or a crawl space. IF the issue is because of the foundation it “could” cost an avgerage of 00.00.
      But repairs can range from 1200.00 to 40000.00.

      If you are on a crawl and the wood floors need replaced, cost would depend on amount of sqare feet and whether the joists need to be supported from underneath.

      Here is what I would do. Call serveral contractors and ask them to bid out the job. The estimate should be free. If they are hungry, they will do it for free and give a little lower price.

      If you do not know who to call, in our area we have an organiztion called, Angies List. It is comprised of reviews from the members and they rate thier workmanship and price for value. ( they only keep good companies that know what they are doing.)

      Visit your local music store and look at the beginner’s books published by Hal Leonard and Mel Bay. Both are excellent tools in learning to play the guitar as there are instructions in chords, maintenance, tuning, and music theory.

      The music theory section comprises of exercises that follow instructions on each string. It would be most beneficial to the student to complete all the exercises, which would lay a foundation of music theory.

      there is no hard-fast rule in taking care of your skin, especially when it comes to aging. skin age does not translate to chronological age. it is how you take care of it that determines what products to use. i have been in the beauty industry for 12 years and have tried every anti-aging product possible in the market/ i even had the chance to help create some. also retail is a good alternative and more cost-effective versus dermatologist / professional treatments.

      sun is the number one cause of aging. it’s good only to an extent, over exposure without protection destroys the foundation of our skin which causes us to wrinkle and look older. there are 3 types of sun rays: UVA (aging) = which penetrates deep into the skin that breaks down it’s very foundation thus causing our skin to age faster, UVB (burning) = which penetrates the surface of the skin that causes it to darken / tan, UVC (cancer) = does not reach us at all, these rays are the shortest and we are lucky that they are 🙂

      anyway, below are some tips and my list of the best products and brands that you can check out and try:

      All states require the seller to disclose known problems when selling a house. You can make the repairs and add the cost into your asking price, or you can let the buyer know about repairs that need to be made and let them deal with it. Most mortgage lenders will require a home inspection and certificate of occupancy. The cost of repairs may also be escrowed by the lender. You may want to contact a civil engineer and discuss what repairs need to be made, whether you do the repairs or the buyer handles the repairs. You didn’t specify if the whole foundation has settled, which may require ‘mud jacking’ or if the house flooring has settled which can be fixed with a house jack in the basement or crawl space.

    Typical Warranty On Foundation Repairs?
    We are close to buying a home with a crawl space constructed of cinder block walls. One wall has a crack in the mortar about halfway up. We obviously didn’t want to deal with major foundation issues, so we had an inspection. The results came back that this is a minor problem and not too uncommon for a house of 53 years. We got a good estimate on epoxy and bracing and some advice on moving water away from the house as a preventative measure.

    The idea is to get this all taken care of by the seller. The odd thing to me was that the man who completed the estimate only offered a 2-3 year warranty on the job. This seemed really low to me, is there an average time for warranties on foundation repairs of this type? Thanks in advance.

      On a cement block support wall, the only descent repair consists of total removal of the mortar in the joints and a replacement of the mortar and any cracked or damaged blocks. Yes, the epoxy is excellent for poured concrete walls, but on block it is not all that good because of the small contact area in square inches that cement blocks have. Remember they are hollow and have only a small contacting surfact that would be bonded to the epoxy. My experience is do not do this epoxy repair.

      I suggest you take a $$ amount in your settlement with the owner and do absolutely nothing to this block wall. Sometimes digging around and disturbing a block wall in a 53 year old structure leads to unforseen problems. Something like ” the cure is worse than the disease”

      Given that this house built 53 years ago is probably very substantial I would do noting with it, although as ive mentioned take the $$ in stead of the repair.

    I really dont have actual pimples, its just red and has acne scarring all over my face.

    The skin itself in most areas is very bumpy looking but sort of smooth to the touch, almost looks wrinkled (not really, but sort of)

      So you need something for oily skin, smoothing, toning, and cover. I don’t want to overwhel you with products but you can choose which ones you want now. You need different products for different issues. If a product clain to do all, you risk damaging your skin.

      Oily Skin:
      The product that I use daily for my oily skin is the Artistry Balancing Skin care system. I use it ever night and sometimes in the morning. It has a cleanser, toner, and lotion specifically for oily skin. Go to http://Arcameda.com and search for balancing. You may also want to get a clay masque if you do not do that already once a week.       Toning and Cover:

      not sure what your asking. the foundation and the floor are 2 different things. is there a chip in the conreat floor under the tile? if so then the mortar will fill that when u set the tile

    How do you fix a crack in cement?
    A tree fell during a storm a couple of weeks ago and left a huge crack on the back patio. Is there any way to fill the crack or cover it? Thanks!

    • ANSWER:
      For serious or exceptionally large concrete crack repairs, you may want to call in a professional, especially if the cracks are appearing around the foundation of your home. A professional can conduct a risk assessment survey to find out if there is structural damage that needs to be addressed.

      Otherwise, fixing the crack itself is not difficult for handy do-it-yourself types.

      For small concrete cracks, 1/8″ (3mm) or less:

      * Use a screwdriver to chip away any flakes or loose concrete.
      * Scour the crack with a wire brush to remove any dirt and debris.
      * Rinse the cracked area using the garden hose.
      * Mix up a small batch of concrete patching compound, available at most hardware stores. Adding a latex additive will allow for the natural expansion of concrete through cold and hot weather.
      * Trowel the compound into the crack using a small putty knife.
      * Smooth the surface while the compound is still moist; concrete compounds begin to set up very quickly.
      * Allow the crack repair to dry thoroughly before walking or driving on the surface.

      While the beginning steps are the same for repairing larger cracks as for small cracks, larger cracks in concrete require the use of a couple of more advanced techniques – caulking or sand packing.

      For large cement cracks, ¼” (6mm) or more:

      * Use a screwdriver to chip away any flakes or loose concrete.
      * Scour the crack with a wire brush to remove any dirt and debris.
      * Rinse the cracked area using a garden hose.
      * If you choose to employ the caulking method, purchase siliconized latex concrete caulk from your local hardware store. Using a caulking gun, force the caulk deep into the crack along the length and breadth of the crack. The caulk will expand to fill the crack, and the nature of the substance will allow for expansion and contraction during weather extremes, cutting down on the frequency of future repairs. The caulking compound will self level so there is no need to smooth the surface, but for large cracks, multiple applications may be needed.
      * If you choose to use the sand packing method, you will need to buy some sand and concrete patching compound, preferably with a latex additive for flexibility. Pour in a layer of sand to help fill the crack, and then apply the patching compound with a trowel. Smooth the surface quickly, and feather the new surface into the existing edges for a finished look.

    Is it possible to get a nice tone on a student/cheap saxophone?
    I really want a smooth, dark, jazzy tone on my alto sax. I feel so limited because of the equipment I have, and I cannot afford a good saxophone which are thousands of dollars.

    Is it all in the technique and breathing, etc?
    Or is a good saxophone, mouthpiece, etc essential to a great tone?
    Any advice on tone? My tone is very airy and not strong.

      Hi, Keira. If the saxophone is in good working order and I have my reed and mouthpiece setup it really doesn’t matter whose name is on the bell- I will get my sound. I’d rather play a less than glamorous horn in excellent mechanical condition which plays in tune regardless of brand or model; in my opinion, shopping for a saxophone is much easier than shopping for a mouthpiece- I try dozens of mouthpieces before making any purchase since the foundation of one’s tone is produced before the air column enters the bore of the saxophone.
      If you are a relative newcomer to playing, hold off on buying an upgrade mouthpiece for roughly a year. Stock mouthpieces (the ones that usually are included with an instrument purchase) are fine for any saxophonist developing the musculature of his/her embouchure since the tip opening is small, enabling one with minimal or developing musculature to squeeze the reed open. When my students want a reed stiffer than a 3 is the usual time to consider an upgrade mouthpiece with a wider tip opening (and usually a bigger chamber, too).
      The best tone building moves I know are long tones (especially low notes); if you practice these in a mirror you get an opportunity to see your embouchure muscles at work, and can evaluate which muscles need to be relaxed or adjusted (this works for hand positioning, too). Once you can play controlled long tones, try using a wall as an acoustic mirror: play long tones initially from three feet away (being attentive to the presence of the sound as it reflects off the wall to your ears), then back away several more feet and try to duplicate the intensity of the reflection produced at the shorter distance- this requires a strong, efficient use of the diaphragm (one such acoustic mirror I used was none other than Notre Dame while on the banks of the Seine- that was work since it was a couple of football fields away!) . If you are a strong “book learner” I’ll recommend one of the Larry Teal volumes which break down elements of technique well; listed from basic to advanced- the Saxophonist’s Workbook, the Saxophonist’s Manual, and the Art of Saxophone Playing. These books are to saxophone technique as Chilton or Haynes manuals are to car care, maintenance, and repair- a thorough breakdown of what it takes to do it well piece by piece.

      What do you do during an earthquake?

      Stay away from glass, windows, outside doors and walls, and anything that could fall, such as lighting fixtures or furniture.

    I work in a building which recently had a car crash into our fence. The contract is very long and is confusing. It seems that the landlord “lessor” is responsible for bearing walls, roof and foundation. I am not clear though on who is responsible for the fence. We came into work this morning and the fence was knocked down due to a car crashing into it.

      The landlord is the lessor. The tenant is the lessee.

      The lease should clearly specify who is responsible for maintaining the fence. If the lease doesn’t specify that the tenant has to pay for any damage incurred, then the landlord is responsible for it.

      But if the fence isn’t actually MENTIONED in the lease (as in, you’re renting a fenced premises) then the landlord has no actual obligation to fix it.

    With homeowners insurance, are closing costs for a new house considered adjusted living expense?
    I lost my house in the Joplin, MO tornado. It was destroyed, leveled, diminished. Foundation was the only thing standing, partially. Would closing cost to buy or build a new house be considered an “adjusted” living expense? To me, I would say yes for the sole factor I normally would not have been in this position if I didn’t lose my house. Thank in advance!

      No.

      Additional living expenses are expenses you incur on a regular basis that are temporarily increased as a result of the loss.

      Closing costs are not a “living” expense. Living expenses are: food, utilities, rent.

      Example: you have to pay rent on an apartment while your home is repaired. At the same time you are paying your mortgage.

    Have you ever bought a house at the auction?
    How complicated is it? Do you need a realtor?

    • ANSWER:
      Its not complicated but be sure you know exactly what your maximum bid is beforehand. The whole point of the auction is to get people excited and bidding more than they otherwise would. Also the contracts are slanted towards the auction company, be sure to review one in advance and be sure you are ok with it. Another thing is the autioneer typicaly cahrges a premium above you auction price so if you win with a bid of say 100,000 they may charge a 3% premium (or whatever) and you end up paing 103,000. Another thing to remember is typically the contract is contingency free, and they require a downpayment at the auction. So if you fail to buy the property for any reason they keep your downpayment, at a minimum, so be absolutely sure you are willing and able to buy the property you bid on. Another thing, be sure to inspect the place beforehand and run comps to find out exaclty what its worth (if your inspection finds problems subtract the cost for those from what you are willing to pay). I’d suggest only bidding on properties in good to great shape unless you really know the fixup costs and can do a really good job determining everything that is wrong with the place (ie will those cracks in the foundation require minor patching, or require major repairs).

      Finally, if you end up winning a property but get a deal that is too good, the seller can still refuse to sell it to you at that price (unless the auction is an “absolute” auction – but very few are), and they will instead try to bargain you up on price – which you may or may not be willing to do.

      Having pointed out all the above problems, there are some pluses. First off the prices are usually (but not always) below market, and sometimes way below market, so you can get some really great deals. As mentioned, the sellers can refuse to sell to you if the deal is too good, but they are paying alot to run the auction and of course the market is weak in most areas right now (very, very weak in some areas) so they really do want to sell the place even if the price is really good.

      So, you can get great deals, but be sure you know what the palce is really worth, and don’t get carried away and bid higher than you wanted to.

      Also, when the bidding is going on – if someone is bidding againist you then put your bids in slowly. Give the other guy time to think about what he is doing so he doesn’t get carried away either.

If you are talking about buying property being foreclosed on (where you typicaly buy it on the courthouse steps) its a whole different set of considerations.       Caleb you do not have lung cancer but you may have sustained lung damage.

      General symptoms of lung cancer

      The symptoms of lung cancer can be

      * Having a cough most of the time
      * A change in a cough you have had for a long time
      * Being short of breath
      * Coughing up phlegm (sputum) with signs of blood in it
      * An ache or pain when breathing or coughing
      * Loss of appetite
      * Fatigue
      * Losing weight

      Less common symptoms of lung cancer

      There are other symptoms of lung cancer that are less common. They are usually associated with more advanced lung cancer. You may have

      * A hoarse voice
      * Difficulty swallowing
      * Swelling of the face caused by a blockage of a main blood vessel from the head (SVCO)
      * Swelling in the neck caused by enlarged lymph nodes
      * Pain or discomfort under your ribs on your right side (from the liver)
      * Shortness of breath caused by fluid around the lungs (called pleural effusion)

      WHAT CAUSES LUNG CANCER?

      Smoking is the number one cause of lung cancer. Cigarette smoke contains more than 4,000 different chemicals, many of which are proven cancer-causing substances, or carcinogens. Smoking cigars or pipes also increases the risk of lung cancer.

      The more time and quantity you smoke, the greater your risk of lung cancer. But if you stop smoking, the risk of lung cancer decreases each year as normal cells replace abnormal cells. After ten years, the risk drops to a level that is one-third to one-half of the risk for people who continue to smoke. In addition, quitting smoking greatly reduces the risk of developing other smoking-related diseases, such as heart disease, stroke, emphysema and chronic bronchitis.

      Many of the chemicals in tobacco smoke also affect the nonsmoker inhaling the smoke, making “secondhand smoking” another important cause of lung cancer. It is responsible for approximately 3,000 lung cancer deaths annually.

      Radon is considered to be the second leading cause of lung cancer in the U.S. today. Radon gas can come up through the soil under a home or building and enter through gaps and cracks in the foundation or insulation, as well as through pipes, drains, walls or other openings. Radon causes between 15,000 and 22,000 lung cancer deaths each year in the United States — 12 percent of all lung cancer deaths are linked to radon.

      Radon problems have been found in every state. The EPA estimates that nearly 1 out of every 15 homes in the U.S. has indoor radon levels at or above the level at which homeowners should take action — 4 picocuries per liter of air (pCi/L) on a yearly average. Radon can be a problem in schools and workplaces, too.

      Because you cannot see or smell radon, the only way to tell if you are being exposed to the gas is by measuring radon levels. Exposure to radon in combination with cigarette smoking greatly increases the risk of lung cancer. That means for smokers, exposure to radon is an even greater health risk.

      Another leading cause of lung cancer is on-the-job exposure to cancer-causing substances or carcinogens. Asbestos is a well-known, work-related substance that can cause lung cancer, but there are many others, including uranium, arsenic, and certain petroleum products.

      There are many different jobs that may involve exposure. Some examples are working with certain types of insulation, working in coke ovens, and repairing brakes. When exposure to job-related carcinogens is combined with smoking, the risk of getting lung cancer is sharply increased.

      A recent study published in the Journal of American Medical Association suggests that particulate matter pollution can cause lung cancer.

      Lung cancer takes many years to develop. But changes in the lung can begin almost as soon as a person is exposed to cancer-causing substances. Soon after exposure begins, a few abnormal cells may appear in the lining of the bronchi (the main breathing tubes). If a person continues to be exposed to the cancer-causing substance, more abnormal cells will appear. These cells may be on their way to becoming cancerous and forming a tumor.

      HOW IS LUNG CANCER DETECTED?

      In its early stages, lung cancer usually does not cause symptoms. When symptoms occur, the cancer is often advanced. Symptoms of lung cancer include:

      * Chronic cough
      * Hoarseness
      * Coughing up blood
      * Weight loss & loss of appetite
      * Shortness of breath
      * Fever without a known reason

This is just one of several ways in which a doctor may take a biopsy sample.

      A growing number of doctors are using a form of CT scan in smokers to spot small lung cancers, which are more likely than large tumors to be cured. The technique, called helical low-dose CT scan, is much more sensitive than a regular X-ray and can detect tumors when they are small.

      More studies on this type of screening will show whether routine screening of smokers and others at risk for lung cancer will save lives.

      If you are diagnosed with cancer, the doctor will do testing to find out whether the cancer has spread, and, if so, to which parts of the body. This information will help the doctor plan the most effective treatment. Tests to find out whether the cancer has spread can include a CT scan, an MRI, or a bone scan.

      You can see a doctor and voice your concerns, but you haven’t really listed any symptoms. Your lung function can be tested as well. And lay off cigarettes or weed. Marijuana has many more carcinogens than cigarettes (and RESIN) that sticks in your lungs.

      Welcome to the Tips and Tricks for Virtual Villagers: The Lost Children, an exciting new game from Last Day of Work and Big Fish Games. Following are some basic strategies to incorporate into play, as well as solutions to the puzzles. So, read at your own risk!

      BASIC STRATEGIES

      Organization
      The first thing to consider is the importance of organization, and naming each villager helps. Select a villager, click the Detail button and change their name to one you can easily remember (like someone you know). This helps you remember who’s who so you can stay on top of each villager.

      Exploration
      Explore early and often! The map’s not very big, but there’s much to be found. Drop your villagers onto anything that looks interesting. Even if it says they don’t have the right skills or skill level, at least you know something is there! Just about every part of your land holds some sort of secret. Keep notes of everything you discover for easy reference later.

      And, in case you didn’t know, you can use the number keys to zip around the island. Keys 1-9 correspond to nine different sections of Isola. You can even pick someone up and quickly whisk them to the desired location.

      Villager Aptitude
      Stay on top of your villagers and their skills — Farming, Building, Research, Healing and Parenting. Initially, make sure you have at least one person working on each of the skills. Some puzzles require a specific level of skill, so you always want to be ready.

      Villagers need to be taught to perform a job. So, keep an eye on them at the start of their “career.” Some have short attention spans and will wander off. Eventually, as they learn their job and reach Trainee level, they will become more automatic at performing it.

      Also, remember to assign skills to each of your villagers on the Detail screen. Note that some children are actually born with an aptitude in one or more skills, inherited from their parents. Keep that in mind when making your assignments, as they will advance more quickly in a skill for which they already have a propensity. You can use this to your advantage, too, by playing matchmaker. If you need more builders, introduce a Master Builder to that special someone.

      Some villagers have certain likes and dislikes, as well. These can effect how your character works. For example, a villager who loves to run around would not be good at researching. In some cases, you may need to experiment with a villager to find their niche vocation.

      Developing Skills
      Skills in Farming are developed by gathering food for your community. At the beginning of the game, you start by dropping a villager into ocean on the west side of the map where they learn to fish. This initially keeps a decent supply of food in the camp. Ultimately, your villagers will learn to climb nearby trees for yummy coconuts, and to plant and harvest crops.

      Building is fairly straightforward. As you progress in technology, new foundations will appear on the map. Just drag and drop a villager onto these and they will begin building. Builders can also repair structures. One of the huts you start with is dilapidated, so repair it when you get a chance. In addition, many secrets are scattered across the map that only Master Builders can uncover. Keep an eye out!

      Research is the easiest skill to develop since it involves just the research table. But, knowing where to invest your points can be tough. Use them on science early because tech points will accrue faster. This way, you can progress in all the other technologies sooner!

      Healing can seem a bit tricky at first, but a few easy ways exist to increase this skill. The first is obvious — heal people! When someone is sick, they wander over by the fire and sit on the ground. You can also tell if someone is sick if you hear them cough. Simply drop your healer onto them and they will be cured. If no one is sick, you can still develop this skill. Around the edge of the map are six plants to study. Drag a villager onto one of these plants and they will begin examining it.     Our existing foundation is out of plumb, should we correct it before connecting new foundation for addition?

      It’s hard to say without seeing it. Some older home foundations were crudely built and not constructed exactly plumb. I would look at it and try to consider if it has moved much recently or if it is still moving out of plumb. Also consider how far off it is. A couple inches over the full height of the wall is not bad if it seems stable and not changing. To rebuild it you would have to jack up the home slightly and build a new wall. That is a big job and saftey should be considered.

      I believe you are looking for a mortar gun. They’re expensive though (like 0 +) and so-so reliability. I’ve always preferred the old grout bags and tuck pointers myself ( and you’re fully outfitted).

      Well, it’s hard to answer without a little more details such as your age and skills and whether you are seeking a long-term or short-term situation so I’ll be addressing the issues with some prefaces.

      If you are over legal age to work in your state, talk to your school about two things: (a) work permit; and (b) a program called Distributive Education that will help you find jobs and give school credit besides your income. It may not be operational in your state but worth asking about.

      If you are under legal age for employment in your state, you’ll have to work for yourself. This is a good thing because you can set your own hours and rate of pay. Your options are to sell products (except alcohol, tobacco or firearms) or provide services (certain restrictions apply locally and nationally).

      If you are looking for a temporary thing and of legal age, check out temporary agencies for factory work, part-time office work, etc. It’s worth making a few phone calls. Don’t waste your time or theirs if you are under age.

      Get your parents on board to help you if you are interested in your own business. For example you may wish to sell online or make and sell crafts or food items or run an errand running business for the elderly or disabled. Make sure you and your parents take extra care for your safety and legal issues like getting permits where needed and not getting into dangerous types of door-to-door sales or what seems like an usual babysitting situation in which you don’t meet the kids, etc. Working a buddy system with friends can also help you both earn money and watch out for each other.

      There are a number of books written for teens and tweens available at your library. I am reading one called “Fast Cash for Kids” to help others with the kind of request you are asking.     My house is about 50 feet from train tracks & trains going by cause shaking (feels similar to a small earthquake). Could this cause structural damage to the house?       I would be concerned. It could be that the tracks were there when your house was built and the builders kept that in mind when designing your house. But I would watch for cracks on your walls and ceiling. And check your foundation at least once a year to make sure it is still sound. Repeated shaking like that will eventually compromise the structure. So keep an eye on it and don’t get behind on repairs.

They covered them and that only lasts for a short while. cracks in drywall are due to settling of the foundation or improper framing. and if you have little one that like to jump jump jump through the house that can start some smaller cracks. jump on the floors aound the cracks and if they vibrate that is your cause either stiffen the framing with blocking(big job) or repair the drywall caulk may help of they are small cracks it with stretch with the crack, worth a try. use poly seam seal or pheno seal– acrylic latex adhesive caulk paintable

      There’s nothing that explicitly prohibits the evolution of immortality. After all, we have all sorts of repair and regeneration mechanisms in place already, and living longer would certainly seem beneficial, as a longer life means more opportunities for reproductive success. So theoretically, there’s nothing really standing in the way. It actually ends up being more of a statistical problem of how easy it is for aging to evolve.

      Say you have a species of immortal organisms capable of dying only by the means you’ve already suggested (predation, starvation, etc.). The longer the organism lives, the more likely the organism will die by any of these methods. If an organism has lived a longer life, it has probably gotten its fair share of injuries, or it has grown bigger and more attractive to predators. Therefore, the probability of survival at an older age is still lower than that of younger ages just due to external factors.

      It may not seem like this should matter to the evolution of aging since everything related to mortality so far has been environmental and nonheritable, but it actually has profound consequences. It introduces age-specific differences in fitness. If all age groups have equal reproductive capacity (older organisms are just as fertile as younger organisms), and survival at each age declines simply due to external factors, what you end up with is greater fitness benefits achieved by reproducing earlier rather than living longer. In other words, if an organism doesn’t have a very high probability of surviving to old age anyway due to external factors, any offspring produced later in life contribute very little to an organism’s overall fitness.

      This is what lays the foundation for intrinsic, as opposed to extrinsic, aging. These organisms can start accumulating mutations that show up later in life, and selection against them will be weak or nonexistent because fitness differences later in life are so small. Alternatively, you can get the evolution of traits that are beneficial earlier in life, but detrimental later in life, but the boost to fitness at a younger age is enough to outweigh the cost to fitness later on. These mutations ultimately lead to deterioration with more advanced age, and ultimately, to death by aging.

      If you can prevent organisms from having differential probabilities of survival between ages, then you can either maintain immortality or expose late-acting deleterious genes to selection, eventually restoring immortality. This is no small task, obviously, because the slightest differences in external factors can cause age-specific differences in fitness. That’s why immortality is not impossible, but highly improbable.

      You’re really asking two questions that are important when you are thinking about beginning real estate investing. First, can you get a mortgage for the whole cost of the house plus fix-up costs. Second, do you have any other options? The answers are “No” and “Yes”. Here are the details:

      First, traditional mortgage lenders will not loan you 100% of the value of the home, and they typically will not loan you money for repairs. There are lenders, known as hard-money lenders, that will do this, but the cost of borrowing their money is very high and, unless you have a lot of rehabbing experience, I do not recommend hard money.

      Second, you DO have other options for beginning real estate investing, and you should seriously consider them. Learn how to easily tie up the deal and then wholesale it to another investor, at least for your first few deals. You can earn really good money finding and wholesaling property to experienced investors, and often they will teach you what they know while you’re building up your credit and bank account.

      There are other methods of investing without using any of your own money or credit, like lease-option and subject-to, but by far the best way of beginning real estate investing, in my opinion, is wholesaling. This will give you a foundation in property values and repair costs in your area before you risk any of your own money.

    At the risk of sounding like a wimp, is it true that as a Boatswain Mate in the Coast Guard that I’ll have to stand out on a bouy rocking back and forth while sharks swim around it? Also, will I have to sit on a board suspended on the side of a ship while it’s plowing through the ocean in order to paint it or make repairs?

    What exactly am I getting myself into?

    • ANSWER:
      ha ha ha…it’s not quite that dramatic!

      BM are the hardest workers in the Navy/Coast Guard, this is true. I have mad respect for the amount of crap they put up with sometimes…bu tnever have I heard of them clinging to buoys in shark infested water or going over the side during full steam. Especially in the coast guard. It is true that you will have to do a lot of chipping grinding and painting, but usually while the ship is pier side…you only have to hang over the ide of the waterside of the ship…and there are several safety devices that keep you from going splat.

      BM’s make the ship run…lets put it that way. You are experts in everything seamanship. From ropes and cables, to navigation and piping (the whistles you hear on the ships)…all the things that lay a good foundation for a tight ship are held together by you. It is a hard life, hardere than most rates, but it’s a good place to lean manual dexterity, strength of body and character, leardership and naval knowlege.

      One perk about Coasties BMs too is the ability to become Coxswains for small boats and taking the VBSS teams to there duties, Having experience in this can help you (if you stay in the military for a career) to someday become the COB, or Chief of the Boat (the highest ranking BM on the ship) …and the job potential in the civilian sector working for law enforcement is great.

      My suggestion is to learn to be flexible (as a person, not physically, though that may help as well) Learn to take critisism (learn to relax when someone is yelling at you…don’t take things really personal…have a quick “rebound” rate), and learn to bust your ass to meet goals and accomplish sometimes what may seem to be impossible tasks…and you will advance fast..and with rank comes privilege.

    Morning drugstore skin care routine and night routine?
    Hello,

    I was wondering how to do a skin care routine. For example, step 1, step 2, step 3, and so on. I want to know if you use a facial cleanser first then Deep Exfoliating Scrub second then toner third then moisturizer. I don’t know which to use first and how to do it morning and night. Do I do the same for morning and night or do I do it differently. Please also recommended some drugstore skin care products. Thanks.

      It would help to know your skin type.

      I have oily acne prone skin and here is my routine, obviously different things work for different people:

      Morning:
      Aveeno Daily Skin Brightening Scrub – this doesn’t have any real “scrub” to it but it’s a great cleanser
      Olay 7 effects moisturizer under my eyes and whole face if I am not using a foundation with SPF or a sunscreen

      Sunscreen is the most important thing you can do for your skin.

      Night:
      Origins Clean Energy Oil Cleanser – washing your face with oil when you have acne and oily skin seems dumb but it’s the best thing I have done for my skin, it’s made a huge difference.
      Body Shop Tea Tree Toner – only during breakouts
      Neutrogena Acne stress control 3 in 1 – this is a bha and is used to chemically exfoliant oily skin, I use this on my T-zone since it is oily and has blackheads
      Neostrata Oil Free smoothing lotion level 3 – this is an aha it is used to exfoliate drier areas or reduce redness, I use this on my cheeks.
      Tea Tree oil – spot treatment on pimples
      L’oreal lash serum – to strengthen my lashes at night.

      Eye cream – Nivea aquasource and Neutrogena eye cream.

      I also sometimes use Estee lauder advanced night repair. When I do it’s before I used the bha and after the toner.

      Make sure if you are going to use a mechanical exfoliator (scrub) that it is very gentle. Many scrubs are too harsh and can cause more damage than good, a chemical exfoliator (aha or bha) will do a better job and be more gentle.

      If you have any more question just add details, I will check back and try to answer.

    Why are houses built on cinder blocks?
    I was just curious about this, but how come builders build houses on cinder blocks? If a bank we have doesn’t give us a loan for a home to buy, why do builders build homes on cinder blocks if no one will be able to buy this home because of the bank not letting us?     • ANSWER:
      I don’t think the cinder block is the problem as much as how they are used and the quality of construction.
      Basement walls and foundations use cinder blocks all the time. At the bottom of the wall is a wider concrete foundation that is below the frost line and wide to spread the load of the home above over a larger area. Much older cheaper homes used posts made of cement or cinder blocks to support the floor beams. That wouldn’t be approved today nor would the home qualify for a new mortgage.
      The foundation under the posts were relatively shallow depending on the heat in the home from allowing the soil to freeze under the house. Frost will lift frozen soil several inches depending on how deep the frost goes. Around here you can see metal fence posts pushed upward out of the ground as much as a foot from the frost pushing up against the collar of concrete attached to the soil line of the post.
      When you buy a house and apply for a mortgage an inspector examines the house to see if it is a worthwhile investment. You don’t own the house until it is paid off. The mortgage company does. They need to make sure the house will last in good repair long enough for you to pay for it. If the house collapses, you leave and they have the mess.
      So if they turn down your app. It not only protects them but you as well. Find another house that will pass the inspection or if you can buy the house on a land contract with the seller and have the deficiencies fixed over time, then buy the house that way. Deduct what it would cost you to bring the house up to a point where it qualifies for a mortgage. If the seller doesn’t agree, find something else.
      Don’t fall in love with the house. It is a business decision more than a love affair.

  44. QUESTION:
    Is anyone familiar with FHA inspections and the cost of repairing the below items
    We are purchasing a home with an FHA loan. The home is only 4 years old and passed a seperate inspection wonderfully.

    First, I’ll say I’m not that familiar with these types of things! But if someone can give me an idea of how easy it would be for my husband/relatives to fix, and a price range (0 or less, etc.)

    SO, the FHA inspector found 2 “problems”. We need to add a gutter extension(s) and we need to fill in dirt on one side of the house (1205 sq. ft house) because there is potential for a wet crawlspace.

    If you need more info, let me know. These aren’t major problems, but our loan requires these repairs to be made. We are hoping the seller will fix these minor repairs, but we are willing to fix them ourselves if the seller does not.

    If anyone is familiar with these types of repair, it would help us plan and budget if we knew how easy and costly it would be for us to make the repairs or how costly and effective it would be to just hire someone.

      Well, a ‘gutter extension’ is just an extra length of gutter pipe to help take the rainwater a little further away from the foundation. You can find them at any hardware or building supply store, usually in at least a couple of colors. They’re simply attached by using a sheet metal screw. You can even find them with curves to help redirect the flow, if need be.

      As to the dirt that needs to be filled in… that really depends on how big the depression, or ‘hole’ is that needs to be filled. Ideally, you want to bring the surface up so that the ground slopes away from the foundation wall.

      Typically, when FHA does an inspection like this, it is the sellers responsibility to correct anything they point out… not the buyers.
      Gutter extensions are easy, so the only real question is how much fill do you need to bring the slope to where it should be. You can call a landscaper for estimates on this. (By the way, the depressions are probably from the ‘backfill’ settling, perfectly normal in a new construction home, but it does need to be taken care of to prevent future damage)

    how can i get flawless skin like really?
    umm my skin is so bad that it looks bad with foundation too. It cakes up and my pimples still show through so do my scars. Its just too red and nasty

    • ANSWER:
      I use to be in the same position. Visit a dermatologist! I promise paying for a visit might be less or equal the amount you pay for random drugstore brand cleansers/scrubs. My derm put me on Retin-A Micro Gel, and if that didn’t work, backup was Accutane. I’ve heard accutane can be dangerous so I’d ask for something less harsh first.

      After having acne for three years (severe acne, and rosacea) I am finally waking up to beautiful skin every morning. Here’s what I do: (btw, I have combo skin, oil t-zone and severely dry cheeks)

      Bedtime:
      -PanOxl 10 bar soap (cleanser, for acne)
      -Retin-A Micro gel (prescription)
      -Estee Lauder’s Advanced Night Repair Serum (I’ve been left with acne scars, and this has faded them dramatically, along with add hydration to my dry cheeks)

      Morning:
      -Splash face with cold water
      -Use Estee Lauder’s Perfectly Clean Toner
      -use Estee Lauder’s Hydronist Serum
      -use Olay Regenerist Moisturizer SPF 15
      then I follow with makeup

      When I have any downtime (homework, on the computer) I use these masks/scrubs:
      -Lemon Juice (fades acne scars, really helps reduce acne appearance and helps with redness)
      -Sugar Scrub (great exfoliator, leaves skin super glowy and bouncy)
      -Jojoba Vitamin E Oil (skin is made of vitamin e, use on dry places)
      -Honey Mask (helps make complexion overall clearer, I try to use this everyday)

    Do you know of any tips or downfalls when buying a foreclosed property?

    • ANSWER:
      I bought my home just over a year ago, and it was a foreclosure. I got a good deal on it, but it was in pretty rough shape. I had to spend some money to fix it up, but in the end I got a good deal. Some things to keep in mind:

      Using an agent is probably a good idea with foreclosures. They are putting their reputation on the line, and their job is to advocate for you. They can also use MLS to find good prices in good neighborhoods, so you aren’t wandering aimlessly or reading papers hoping to find a good deal before anyone else does. A good agent does their homework on the property, and will tell you if anything is amiss. Ask around to see if anyone can recommend a good one.

      Liens and other claims on the property should be cleared before you even hear about it. When you go to close on the sale, the title broker will set you up with title insurance as part of standard closing procedure. The title insurance protects against any liens or other claims on the property.

      I would recommend getting a handy and knowledgable friend or family member to check out the property with you before you make a bid. Look at everything, and take it all into account. How old is the furnace and water heater? How is the plumbing? Is the foundation sound and free of cracks? What shape are the windows in? How is the brick work or siding? How much will you have to spend to fix the place up, and how much are you saving on buying this foreclosure? Make sure it’s a good deal.

      Plan ahead. If you are going to need to do a lot of repairs, make sure the place is liveable before you plan to move in. If you have to do repairs, how long will they take? What can be worked on while you live there, and what is high priority and needs to be done immediately?

    I have Interstitial Cystitis. How are others coping with it and/or being treated for it if at all?

    • ANSWER:
      Causes
      The cause of interstitial cystitis is unknown, though several theories have been put forward (these include autoimmune, neurologic, allergic and genetic). Regardless of the origin, it is clear that IC patients struggle with a damaged mucin, aka the GAG layer, aka bladdering lining. When this protective coating is damaged (perhaps via a UTI, excessive consumption of coffees or sodas, traumatic injury, etc.), urinary chemicals can “leak” into surrounding tissues causing pain, inflammation and urinary symptoms. Oral medications like Elmiron and medications which are placed directly into the bladder via a catheter work to repair and hopefully rebuild this damaged/wounded lining, allowing for a reduction in symptoms.

      Recent work by the University of Maryland, Baltimore indicates that genetics are a factor in, and may even (in some cases) be the cause of IC. Two genes, FZD8 and PAND, are associated with the syndrome. FZD8, at gene map locus 10p11.2, is assocated with an antiproliferative factor secreted by the bladders of IC patients which “profoundly inhibits bladder cell proliferation,” thus causing the missing bladder lining. PAND, at gene map locus 13q22-q32, is associated with a constellation of disorders (a “pleiotropic syndrome”) including IC and other bladder and kidney problems, thyroid diseases, serious headaches/migraines, panic disorder, and mitral valve prolapse.

      Symptoms
      It may well be that the symptoms of interstitial cystitis have multiple causes, and IC is actually several syndromes which will eventually be discerned. For example, patients with Hunner’s Ulcers are believed to be the most advanced cases. They have larger “wounds” in the bladder that are much more difficult to treat. It is estimated that only 5 to 10% of patients have these ulcers. Far more patients may experience a very mild form of IC, in which they have no visible wounds in their bladder, yet struggle with symptoms of frequency, urgency and/or pain. Still other patients may have discomfort only in their urethra, while others struggle with pain in the entire pelvis. Some patients may experience pelvic floor tightness and dysfunction, while others have normal muscle tone.

      Often the symptoms of IC are misdiagnosed as a “common” bladder infection (cystitis), however unlike cystitis, IC has not been conclusively proven to be a bacterial infection and the mis-prescribed treatment of antibiotics is ineffective. The symptoms of IC may also initially be attributed to prostatitis and epididymitis (in men) and endometriosis and uterine fibroids (in women).

      Diagnosis
      Diagnosis has been greatly simplied in recent years with the development of two new methodologies. The Pelvic Pain Urgency/Frequency (PUF) Patient Survey, created by C. Lowell Parsons, is a short questionnaire that will help doctors identify if pelvic pain could be coming from the bladder. The KCL Test, aka the Potassium Sensitivity Test, uses a mild potassium solution to test the integrity of the bladder wall. Though the latter is not specific for IC, it has been determined to be helpful in predicting the use of compounds, such as pentosan, which are designed to help repair the GAG layer. The previous “gold standard” test for IC was the use of hydrodistention with cystoscopy. Researchers, however, determined that this visual examination of the bladder wall after stretching the bladder was also not specific for IC and that the test, itself, can contribute to the development of small glomerulations (aka petechial hemorrhages) often found in IC. Thus, a diagnosis of IC is one of exclusion, as well as a review of clinical symptoms.

      Diet
      The foundation of therapy is a modification of diet to help patients avoid those foods which can will further irritate the damaged bladder wall. Common offenders include coffees, teas, herbal teas, green teas, all sodas (particularly diet), concentrated fruit juices, multivitamins, monosodium glutamate, chocolate, and potassium-rich foods such as bananas. Most IC support groups and many urology clinics have diet lists available.

      The problem with diet triggers is that they vary from person to person: the best way for a person to discover his or her own triggers is to use an elimination diet. This is where someone cuts out all foods except the basics (e.g. potatoes, bread, rice, water) and then introduces new foods one at a time. Trying to discover which foods are one’s own triggers without the use of an elimination diet is like trying to do a scientific experiment whilst altering 10 variables all at once.

      Bladder coatings
      As recently as a decade ago, treatments available were limited to the use of astringent instillations, such as clorpactin or silver nitrate, designed to kill infection and/or strip off the bladder lining. In 2005, our understanding of IC has improved dramatically and these therapies are now no longer done. Rather, IC therapy is typically multi-modal, including the use of a bladder coating, an antihistamine to help control mast cell activity and a low dose antidepressant to fight neuroinflammation.

      The two US FDA approved therapies for IC have had recent setbacks in various research studies. Oral Elmiron (aka pentosan polysulfate) is believed to provide a protective coating in the bladder, however data released in late 2005 by Alza Pharmaceuticals suggests that 84% of Elmiron is eliminated, intact, in feces. Another 6% is excreted via urine. Reference: Metabolism of [3H]pentosan polysulfate sodium (PPS) in healthy human volunteers. Xenobiotica. 2005 Aug;35(8):775-84. In addition, the NIH funded ICCTG study of pentosan revealed results only slightly better than placebo.

      DMSO, a wood pulp extract, is the only approved bladder instillation for IC yet it is much less frequently used in urology clinics. Research studies presented at recent conferences of the American Urological Association by C. Subah Packer have demonstrated that the FDA approved dosage of a 50% solution of DMSO had the potential of creating irreversible muscle contraction. However, a lesser solution of 25% was found to be reversible. Long term use is questionable, at best, particularly given the fact that the method of action of DMSO is not fully understood.

      More recently, the use of a “rescue instillation” composed of elmiron or heparin, cystistat, lidocaine and sodium bicarbonate, has generated considerable excitement in the IC community because it is the first therapeutic intervention that can be used to reduce a flare of symptoms. Published studies report a 90% effectiveness in reducing symptoms.

      Another bladder coating treatment is that of Cystistat(TM) which consists primarily of sodium hyaluronate. It is believed to replace the deficient GAG layer on the bladder wall. Like most other intravesical bladder treatments, this treatment requires the patient to lie for 20 – 40 minutes, turning over every ten minutes, to allow the chemical to ‘soak in’ and give a good coating, before it is passed out with the urine.

      Pelvic floor treatments
      Pelvic floor dysfunction may also be a contributing factor thus most major IC clinics now evaluate the pelvic floor and/or refer patients directly to a physical therapist for a prompt treatment of pelvic floor muscle tension or weakness. Pain in the bladder and/or pelvis can trigger long term, chronic pelvic floor tension which is often described by women as a burning sensation, particularly in the vagina. Men with pelvic floor tension experience referred pain, particularly at the tip of their penis. In 9 out 10 IC patients struggling with painful sexual relations, muscle tension is the primary cause of that pain and discomfort. Tender trigger points, small tight bundles of muscle, may also be found in the pelvic floor.

      Pelvic floor dysfunction is a fairly new area of specialty for physical therapists world wide. The goal of therapy is to relax and lengthen the pelvic floor muscles, rather than to tighten and/or strengthen them as is the goal of therapy for patients with incontinence. Thus, traditional exercises such as Kegels, can be helpful as they strengthen the muscles, however they can provoke pain and additional muscle tension. A specially trained physical therapist can provide direct, hands on, evaluation of the muscles, both externally and internally. While weekly therapy is certainly valuable, most providers also suggest an aggressive self-care regimen at home to help combat muscle tension, such as daily muscle relaxation audiotapes, stress reduction and anxiety management on a daily basis. Anxiety is often found in patients with painful conditions and can subconsciouly trigger muscle tension.

      Pain control
      Pain control is usually necessary in the IC treatment plan. The pain of IC has been rated equivalent to cancer pain and should not be ignored to avoid central sensitization. The use of a variety of traditional pain medications, including opiates, is often necessary to treat the varying degrees of pain. Complementary therapies such as acupuncture, massage, and biofeedback are also beneficial to some patients. Even children with IC should be appropriately addressed regarding pelvic pain, and receive necessary treatment to manage it.

      Electronic pain-killing options include TENS (a machine connected to sticky pads which one palces on their body at certain pressure points; the tens machine sends electrical impulses to the skin, using the human body as an ‘earth’). PTNS stimulators have also been used, with varying degrees of success. This is similar to a TENS treatment, except a needle is used rather than sticky pads.

      Other treatments
      Bladder distentions (a procedure which stretches the bladder capacity, done under general anaesthetic) have shown some success in reducing urinary frequency and giving pain relief to patients. However, many experts still cannot understand precisely how this can cause pain relief. Unfortunately, the relief achieved by bladder distentions is only temporary (weeks or months) and consequently, it is not really viable as a long-term treatment for Interstitial Cystitis: it is generally only used in extreme cases.

      Surgical interventions are rarely used for IC. Neurostimulation techniques are not FDA approved for IC.
